Protocol Review Forms

The U.S. Army Medical Materiel Development Activity (USAMMDA) performs a Sponsor Protocol Review Board (PRB) review prior to Institutional Review Board (IRB) submission of protocol and related documents. This review begins with the submission of a final protocol package developed by the Integrated Product Team (IPT)/Working Group (WG) and relevant USAMMDA personnel.

Please complete the following 3 steps to request sponsor protocol review and approval:

  1. Fill in the fields below and submit request.
  2. Complete Form .5.2.27, Protocol Package Requirements List. This form is a checklist that will be used to determine if the protocol package is complete and meets the requirements to initiate sponsor review.
  3. Submit completed Form .5.2.27 and all of the required protocol package documents, as listed in the form, to the following e-mail address: usarmy.detrick.medcom-usammda.mbx.sponsor-protocol-review@health.mil

Please do not include information that should otherwise be protected.

Upon receipt of the form and all documents, the protocol review will be initiated. The Regulatory Project Coordinator will notify you if the package is rejected due to incomplete submission, or if it is accepted with the date and time of the Protocol Review Board Meeting.

Submission of the protocol review request to the Regulatory Project Coordinator does not guarantee approval of the protocol.
